Nasza Fundacja świadczy pomoc na rzecz osób będących ofiarami wypadków komunikacyjnych, w szczególności poprzez:
- doradztwo w zakresie pomocy prawnej i rehabilitacyjnej
- dofinansowanie kosztów: rehabilitacji, zakupu sprzętu medycznego, ortopedycznego i protez, zakupu leków oraz środków pielęgnacyjnych, szkolenia, a także przekwalifikowania zawodowego, pomocy prawnej.
Dofinansowanie obejmuje z reguły część kosztów, w zależności od indywidualnej sytuacji osoby zwracającej się o pomoc. Dofinansowanie następuje w formie promesy, tj. jednostronnego zobowiązania Fundacji do wypłaty środków po spełnieniu przez poszkodowanego pewnych warunków (np. zgromadzenia pozostałej części kosztów, przedstawienia rachunku potwierdzającego poniesienie kosztów). Dla Fundacji stanowi to gwarancję, iż przekazane środki nie zostaną wykorzystane w innym celu, zaś dla poszkodowanego - gwarancję, iż w razie spełnienia warunków określonych w promesie Fundacja zobowiązana będzie do wypłaty środków. Na kwoty przewidziane w promesie Fundacja tworzy rezerwy finansowe.
Wymagane dokumenty:
1.pismo zawierające prośbę o pomoc z uzasadnieniem,
2. dokumenty z wypadku (z postępowania karnego) potwierdzające, że urazy czy schorzenia są wynikiem wypadku oraz kto był jego sprawcą,
3. informacji o stanie działań odszkodowawczych,
4. ksero wypisów szpitalnych powypadkowych,
5. zaświadczenia o dochodach za ostatnie 3 miesiące (z uwzględnieniem wszystkich dochodów tzn. wynagrodzenia, zasiłki, renty, emerytury + ewent. zajęć na wynagrodzeniu) oraz oświadczeniem o liczbie osób pozostających we wspólnym gospodarstwie domowym,
6. informacji z ośrodka pomocy społecznej o sytuacji materialno - bytowej rodziny,
7. informacji czy i kiedy oraz na co ubiegał/-a się Pan/Pani o pomoc ze środków Państwowego Funduszu Rehabilitacji Osób Niepełnosprawnych i Powiatowego Centrum Pomocy Rodzinie,
8. informacji czy zwracał/-a się Pan/Pani o wsparcie finansowe do innych organizacji, jeśli tak, to jakie organizacje udzieliły wsparcia, a jakie odmówiły pomocy,
9.ksero orzeczenia o stopniu niepełnosprawności.
Ponadto jeżeli ubiega się Pan/Pani o niżej wymienioną pomoc należy dołączyć dodatkowo następujące dokumenty:
Dofinansowanie pobytu na turnusie rehabilitacyjnym:
1.zaświadczenie kwalifikujące na turnus rehabilitacyjny z informacją o terminie i kosztach leczenia;
2.zaświadczenie z Powiatowego Centrum Pomocy Rodzinie z informacją o wysokości dofinansowania przyznanego do wyżej wymienionego turnusu.
Dofinansowanie rehabilitacji indywidualnej:
1.zaświadczenie lekarskie o stanie zdrowia z informacją o potrzebie rehabilitacji indywidualnej, ilości zabiegów w tygodniu, oraz dokument, że nie jest ona finansowana przez Narodowy Fundusz Zdrowia;
2.zaświadczenie od rehabilitanta z informacją o kosztach związanych z rehabilitacją.
Dofinansowanie sprzętu medycznego (sprzęt rehabilitacyjny, aparaty słuchowe, okulary korekcyjne, sprzęt ortopedyczny, protezy):
1.zaświadczenie lekarskie o stanie zdrowia z informacją o potrzebie zakupu danego sprzętu medycznego;
2.wstępny kosztorys sprzętu (np. ksero faktury proforma);
3.pismo z Narodowego Funduszu Zdrowia i z Powiatowego Centrum Pomocy Rodzinie z informacją, czy wyżej wymienione instytucje dofinansowują zakup sprzętu, jeżeli tak to w jakiej kwocie.
Dofinansowanie leczenia:
1.zaświadczenie lekarskie o konieczności zakupienia leków i środków pielęgnacyjnych,
2.zaświadczenie z apteki z informacją o kosztach leczenia;
W wyjątkowych wypadkach świadczymy również pomoc na rzecz osób poszkodowanych będących jednocześnie sprawcami takich wypadków, jednakże pod warunkiem, że: osoby te nie znajdowały się w chwili wypadku w stanie po użyciu alkoholu lub środków odurzających, inne osoby nie odniosły w wyniku takiego wypadku obrażeń powodujących naruszenie czynności narządu ciała lub rozstrój zdrowia.

Our Foundation provides assistance to persons who are victims of traffic accidents, in particular by:
- Advice on legal aid and rehabilitation
- Financing costs: rehabilitation, purchase of medical equipment, orthopedic
- appliances and prostheses, the purchase of medicines and care products, training, and outplacement assistance.
Grants usually cover part of the cost, depending on the individual circumstances of the person requesting assistance. Funding is in the form of promissory note, ie a unilateral commitment of the Foundation to pay the victim after meeting certain conditions (eg, meeting the rest of the cost account to submit evidence of costs incurred). For the Foundation provides a guarantee that the transferred funds are not used for any other purpose, and the victim - the assurance that if the conditions specified in the promise Foundation will be obliged to pay the funds. At the amounts provided for in the promise Foundation creates financial reserves.
1th.letter containing the request for assistance on the ground, 2 documents from the case (the prosecution) evidence that the injuries or illnesses are the result of an accident and who was the perpetrator,
3rd information about the state of compensation,
4th Photocopying hospital discharge following an accident,
5th proof of income for the last three months (including all income, ie salary, allowances, pensions resp. on the salary schedule), and a statement of the number of people remaining in the household,
6th information from the center of social assistance on the situation of material - household family
7th information if and when and on what ran / s to you for aid from the State Fund for Rehabilitation of Persons with Disabilities and the District Family Assistance Centre
8th information or pointed / s to you for financial support to other organizations, if so, what organizations offered support and assistance they refused,
9thPhotocopying judgments about the degree of disability.
In addition, when a person you are with the following support must be accompanied by the following additional documents:
Grants Camp Rehabilitation stay:
1.certificate canp eligible for rehabilitation with information about the time and cost of treatment;
2.certificate the District Family Assistance Center with the information about the amount of subsidies granted to the said stay.
Financing of rehabilitation equipment:
1.certificate medical state of health with information about the need for rehabilitation of the individual, the number of treatments per week, and the document that it is not funded by the National Health Fund;
2.certyficate of rehabilitation with information about the costs associated with rehabilitation.
Funding for medical equipment (hardware and rehabilitation, hearing aids, corrective, orthopedic equipment, prostheses)
1.certificate medical state of health with information about the need for the purchase of medical equipment;
2.preliminary estimate of equipment (eg Photocopying pro forma invoice);
3.letter from the National Health Fund and the District Family Assistance Center with the information that these institutions provide financial purchase of equipment, if so in what amount.
Funding of treatment:
1.certificate medically necessary funds to purchase medicines and care,
2.certificatethe pharmacy with the information about the cost of treatment;
